09. Clozapine Plasma Levels
Key Points
- Use of clozapine levels is the standard of care.
- Efficacy is unlikely to be seen at plasma clozapine levels under 350 ng/mL.
- If tolerated, plasma levels can be pushed as high as 1000 ng/mL in search of greater efficacy. However, many patients may develop tolerability issues at higher plasma levels.
- It should be noted that the development of seizures at higher levels is not a reason to discontinue clozapine, but instead to add divalproex and continue the titration.
