06. Stevens-Johnson Syndrome and Lamotrigine
Key Points
- If any sign that indicates Stevens-Johnson syndrome is present:
- Stop lamotrigine without taper
- If the rash lacks any of the features suggesting SJS:
- Some clinicians lower the dose one step and watch very closely until the rash is clearly gone
- Then after one week, they resume titration with smaller increments and more slowly
- A key literature review reports that lamotrigine can be successfully restarted even in patients with features of SJS
- Starting dose: 5 mg
- Increase dose 5 mg every two weeks
