05. Agitation Requiring IM Antipsychotics
Key Points
Recommendations:
- IM haloperidol 5 mg plus lorazepam 2mg is still the treatment of choice for rapid, safe, cost-effective treatment of severe agitation in schizophrenia.
- It may be repeated once or twice in 30-60 minutes.
- Avoid chlorpromazine IM:
- High risk of sudden, serious hypotension and same efficacy as haloperidol
- Oral PRN antipsychotics are often used but are usually unnecessary for less severe agitation:
- A benzodiazepine is just as effective
