Now, let’s look at a new expert consensus statement from the European College of Neuropsychopharmacology, their obsessive-compulsive disorders group. In this publication, there is 1 finding that is strongly emphasized: A more rapid initiation of treatment after detection of symptoms is associated with better outcomes. The longer it takes to get there with treatment, the greater one observes in association increased comorbidity and disability. The implication is that treating early can decrease that comorbidity and disability and get better outcomes. But they also emphasized this is an extrapolation which is not yet demonstrated in longitudinal studies. The average duration of untreated illness is 2 to 3 years in children and more in adults. Looking at brain imaging data, there are changes in brain structure with age. So, the implication is that OCD is evolving, but that’s not yet demonstrated in longitudinal data.
